    Reporting Form

    Thank you for reporting an incident or near miss. Such reporting is essential for maintaining a safe work environment, safeguarding occupants, and preventing future incidents.

    Examples of incident or near-miss reporting include:

    • Incident: A worker slips and falls on a wet floor, resulting in a sprained ankle.
    • Near Miss: A worker slips on a wet floor but manages to regain balance and avoid falling.
       
    • Incident: A piece of equipment malfunctions, causing minor burns to a worker.
    • Near Miss: A worker notices a malfunctioning piece of equipment and reports it before anyone is injured.
       
    • Incident: A worker is exposed to hazardous chemicals due to improper handling, resulting in skin irritation.
    • Near Miss: A worker almost spills a container of hazardous chemicals but manages to prevent it from happening.

    Thank you for taking the initiative to report a hazard or unsafe condition that you have observed on campus. Hazard reporting is essential in identifying and documenting potential risks or hazardous conditions in the workplace. Such reporting could proactively address these risks and prevent accidents, injuries, and illnesses.

    Examples of hazard or unsafe condition reporting include:

    • Physical Hazard: A report of a loose handrail on a staircase that poses a falling hazard.
    • Chemical Hazard: A report of a spill of a hazardous chemical substance in a laboratory.
    • Biological Hazard: A report of mould growth in a building that could lead to respiratory issues.
    • Unsafe Condition:
      • A report of a wet floor without a warning sign, posing a slip hazard.
      • A report of a malfunctioning machine or equipment that could cause injury to operators.
      • A report of missing or damaged safety equipment, such as fire extinguishers or first aid kits.
